What actually fails performance assessments
Performance assessments rarely fail on exotic grounds. The same five evidence gaps carry most findings, year after year. Each row links to the Quality Area it sits under and the product that closes it.
Tools that do not cover unit requirements, are not contextualised to your learner cohort, or were never reviewed before use. This is the single most common thread in adverse findings.
